Please provide TWO documents as specified below:
Primary: 1 or 2 of the following
- Proof of Purchase (For example: Bill of Sale, Buyer's Order, Purchase Agreement, Retailer Installment Sales Contract, or other)
- Must contain the buyer’s name and address, seller’s name and address, VIN, make, model, year, mileage, and purchase price.
- Must be signed by both the buyer and the seller.
- Title (front and back)
- Must be issued in the vehicle owner’s name.
- Does NOT include the Application for Title.
Secondary: Must receive 1 of the Primary AND 1 of the following
- Current, valid Registration (can include temporary if the primary is also received)
- Current, valid Insurance card
